Preimplantation Genetic Testing for Structural Rearrangements

What is PGT- SR (Preimplantation Genetic Testing for Structural Rearrangements)?

PGT-SR is a test that screens embryos for inherited chromosomal structural rearrangement before transferring them. This increases the chances of a successful pregnancy. Testing is often done after multiple miscarriages or if a child is born with chromosomal issues. Parents usually appear healthy but may have structural changes in their chromosomes that they are unaware of. Their condition increases their chances of producing embryos with the wrong chromosome number or structure.

Who should be offered PGT-SR?

The PGT-SR test is helpful for those with chromosomal rearrangements, such as inversion, reciprocal translocation, Robertsonian translocation, and so on, identified by specialized genetic testing like karyotyping. It detects inappropriate chromosome copy numbers, which means gains or losses of chromosomes or parts of the chromosomes. By transferring embryos with the correct chromosomal content, it enhances the possibility of a healthy pregnancy.

How is PGT-SR done?

This process involves testing embryos derived through IVF and transferring the ones that have the proper amount of chromosomal material after selecting them. The embryo is biopsied when it has reached the blastocyst stage of development, five days in culture and composed of hundreds of cells.
The biopsy takes 5 to 7 cells from the external layer of the embryo, called the trophectoderm, without contacting the inner cell mass destined to produce the fetus.Genetic material from the embryo is analyzed. After the biopsy, embryos are frozen. It is also possible to establish the existence of unbalanced chromosomal rearrangements by ascertaining partial/sub-chromosomal gains or losses in genetic material.
Based on the results of PGT-SR, the good-quality embryos are thawed and transferred to the patient’s uterus in a subsequent cycle.

What are the advantages of PGT-SR?

PGT-SR is a key analytical technique suggested for individuals (male or female) with a chromosome rearrangement (structural or numerical). The test increases the likelihood of successful pregnancy after IVF and reduces the chance of having severe conditions in the baby.
PGT-SR reduces the chances of miscarriage or a pregnancy with a genetically abnormal fetus.
It reduces the stress of multiple failed IVF cycles and early miscarriages often experienced by carriers of a structural rearrangement.
